CONSENT FORM - OABF BENEFICIARY TO BECOME A GROUP MEMBER OF LADY GROVER’S FUND

To become an Officers’ Association Benevolent Fund (OABF) Group member of the Lady Grover’s Fund (LGF), your agreement and consent is needed when completing the OABF Group membership application form. You will be giving consent for OABF to provide your name, date of birth and contact information to LGF, so they can set up and manage your membership of LGF.

The LGF will then be responsible for managing and safeguarding this data but will only use it to manage your membership in accordance with its Privacy Notice that you will find here.
